- Imagine Wellness
Medication can make a real difference for anxiety, especially when worry or panic is strong enough to get in the way of daily life. It doesn’t change who you are, and it isn’t a sign of weakness. Most anxiety medications work by adjusting brain chemistry that affects mood and the body’s stress response, which can lower anxiety enough for therapy and everyday coping skills to take hold.
Medication rarely works best on its own. For many people, the strongest results come from combining medication with therapy such as CBT. Medication can ease symptoms while therapy teaches skills that last after treatment ends. Some people with milder anxiety improve with therapy alone, and a prescriber can help you decide whether medication makes sense for you.
A prescriber chooses a medication based on your type of anxiety, other health conditions, other medications you take, past responses to treatment and your preferences about side effects.

Understanding Your Options
Selective serotonin reuptake inhibitors (SSRIs) are usually the first medication prescribers try for generalized anxiety disorder, panic disorder and social anxiety disorder. They are antidepressants, but they treat anxiety on their own, whether or not someone has depression. SSRIs increase the availability of serotonin, a brain chemical involved in mood and fear responses.
SSRIs take time. Most people notice some change within 2 to 6 weeks, and the full benefit can take a few months. Some people feel more jittery in the first week or two, which usually fades. Prescribers often start with a low dose and raise it slowly to limit side effects.
Serotonin-norepinephrine reuptake inhibitors (SNRIs) work on two brain chemicals, serotonin and norepinephrine. Prescribers often use them for generalized anxiety disorder, and they may be the next option when an SSRI doesn’t help enough or causes side effects. SNRIs follow a similar timeline to SSRIs, and venlafaxine can raise blood pressure, so prescribers usually monitor it.
Buspirone is approved specifically for generalized anxiety disorder. It is not an antidepressant or a sedative, and it does not carry the dependence risk of benzodiazepines. People usually take it two or three times a day, and it takes about 2 to 4 weeks to work, so it does not help in the middle of a panic attack. Prescribers sometimes add it to an SSRI when one medication alone isn’t enough.
Benzodiazepines work quickly, often within an hour, which makes them useful for short-term relief of severe anxiety or panic. Prescribers usually limit them to short periods or occasional use. Regular use can lead to physical dependence, and stopping suddenly can cause withdrawal symptoms, so a prescriber should guide any change in dose. Benzodiazepines cause drowsiness and slowed reactions, and they can be dangerous combined with alcohol, opioids or other sedating medications.
Hydroxyzine is an antihistamine that some prescribers use for short-term anxiety relief. It works within about an hour, does not carry a dependence risk and often causes drowsiness.
Beta blockers such as propranolol calm the physical symptoms of anxiety, like a pounding heart or shaking hands. Prescribers sometimes use them off-label for performance anxiety, such as before a presentation. They do not treat the worry itself, and they are not safe for everyone, including some people with asthma.
What to Expect
Fast-acting medications such as benzodiazepines and hydroxyzine work within hours, while SSRIs, SNRIs and buspirone take weeks. That delay is one of the most common reasons people stop too soon. If a medication hasn’t helped after a fair trial, your prescriber can adjust the dose or try a different option. Many people need more than one try to find the right fit.
When a medication works, prescribers often recommend staying on it for 6 to 12 months after symptoms improve before talking about tapering off.
Staying Safe
Most side effects of SSRIs and SNRIs are mild and ease within a few weeks. Side effects that last, or that make it hard to stay on the medication, are worth raising with your prescriber, who can often adjust the dose or timing. A few safety basics apply to every anxiety medication:
Finding Balance
Medication and therapy both work for anxiety, and they work in different ways. For moderate to severe anxiety, the combination often works better than either one alone.
Medication
Medication can lower symptoms within weeks, but anxiety may return after stopping.
Therapy
Therapy, especially CBT, takes effort and practice, and its benefits tend to hold up after treatment ends.

No single medication is best for everyone. SSRIs are usually the first choice for long-term anxiety, and a prescriber picks a specific option based on your symptoms, health and history.
SSRIs and SNRIs usually take 2 to 6 weeks to start helping, and buspirone takes about 2 to 4 weeks. Benzodiazepines and hydroxyzine work within hours.
They depend on the medication. SSRIs and SNRIs can cause nausea, headache, sleep changes and sexual side effects, which often ease over time. Benzodiazepines cause drowsiness and can lead to dependence with regular use.
You can, but many people do better with both. Therapy builds skills that keep working after medication stops.
Stopping suddenly can cause withdrawal or discontinuation symptoms. Talk with your prescriber, who can plan a gradual taper.
