TIPP Skills: A DBT Tool for the Moment Anxiety Spikes

When anxiety climbs fast, your body can get ahead of your thinking. Your heart races, your breathing turns shallow, and calming thoughts don’t seem to land. TIPP is a set of four skills from dialectical behavior therapy designed for that moment.

What TIPP Stands For

TIPP stands for temperature, intense exercise, paced breathing and paired muscle relaxation. The four skills are part of the distress tolerance skills taught in dialectical behavior therapy (DBT). Distress tolerance is about getting through a hard moment without making it worse.

TIPP works through the body rather than through reasoning. That is why it can help when you are too worked up to think a problem through.

Why Body-Based Skills Help When Anxiety Spikes

Strong anxiety sets off the body’s alarm system. Your heart beats faster, your breathing speeds up and your muscles tense. Trying to talk yourself down at that point often fails, because the alarm is louder than the thought.

TIPP skills aim to slow the physical response first. Once your body settles a little, it gets easier to use other coping skills or decide what to do next.

Temperature

Cold water on the face is the most common version. Hold your breath and put your face in a bowl of cool water, or hold a cold pack wrapped in a cloth over your eyes and cheeks, for about 30 seconds. Cold on the face can trigger the body’s dive response, which slows the heart rate. Many people notice a shift within a minute or two.

Check first: because this can lower your heart rate, talk with a doctor before using it if you have a heart condition, low blood pressure or an eating disorder, or if you take medication that affects your heart rate.

Intense Exercise

A short burst of hard movement gives the energy anxiety creates somewhere to go. Climb stairs briskly, do jumping jacks or walk fast for 10 to 20 minutes, at whatever level your body can safely handle. The goal is not fitness. It is to use up some of the adrenaline your body has released.

If you have a medical condition that limits exercise, ask your doctor what level is safe for you.

Paced Breathing

Slow your breathing so each exhale is longer than the inhale. For example, breathe in for about four counts and out for about six, and keep going for a few minutes. A longer exhale is one of the simplest ways to signal your nervous system to settle. If counting feels like one more thing to get right, just make the out-breath slower than the in-breath.

Paired Muscle Relaxation

As you breathe in, tense one muscle group, such as your hands or shoulders, for a few seconds. As you breathe out, let it go and notice the difference. Move through a few areas of your body. Pairing the release with a slow exhale reminds your body what relaxed feels like, which is hard to recall in the middle of a spike.

When TIPP Helps and When It Isn’t Enough

TIPP is a short-term tool. It is meant for moments when distress is very high, not as a way to manage anxiety day to day. If you find you need it often, or the spikes keep getting stronger, that is useful information to bring to a therapist.

Ongoing anxiety usually responds best to care that looks at what is driving it, and ongoing support for anxiety at Imagine Wellness can combine therapy, skills practice and medication management when it fits. DBT is one of several approaches used for anxiety, and it can help to see how DBT and CBT compare side by side.

Practicing DBT Skills With Support

Skills like TIPP are easier to reach for under stress when you have practiced them while calm. At Imagine Wellness, DBT skills can be part of an individualized treatment plan at our outpatient centers in Peachtree Corners and Fayetteville. If anxiety spikes are making daily life harder, call (770) 270-3681 to talk with our admissions team about what kind of support could fit.

Frequently Asked Questions

What does TIPP stand for in DBT?

Temperature, intense exercise, paced breathing and paired muscle relaxation. They are distress tolerance skills for moments of very high emotion.

How fast do TIPP skills work?

Many people notice a change within a few minutes, especially with the temperature skill. How much they help varies from person to person.

Can TIPP skills stop a panic attack?

They may soften or shorten one for some people, but they are not a guaranteed fix. If panic attacks happen often, talk with a mental health professional.

Are TIPP skills safe for everyone?

Temperature and intense exercise both affect heart rate. Check with a doctor first if you have a heart condition, an eating disorder or another medical condition.
